In order to ensure optimum success and harmony within each program, Dr. Kolp personally evaluates each applicant over a two stage application process. To be considered, applicants must complete a series of psychological measures and medical questionnaires as well as undergo a full psychiatric evaluation with Dr. Kolp. Application Process

    • Step 1: Online Application

To begin the application process at the Kolp Institute, we first require all potential clients to complete a lifestyle questionnaire, a series of psychological measures, a medical questionnaire, and an informed consent. All required forms can be found on our Questionnaires and Psychological Measures page. The fee for this initial assessment is $25 and should be paid at the time you submit your application materials to the Kolp Institute. Application materials received without the application fee will not be processed. This fee will apply to the cost of your individual psychiatric evaluation upon acceptance into the second phase of the application process. You will provide us with your basic demographic information; your personal medical and psychiatric history; your family medical and psychiatric history; your exercise and eating habits; and your lifestyle habits, including alcohol and drug consumption. If you are contraindicated for our program (not accepted), we will advise you via email and letter, and retain your application as part of your medical record in our office for two years. In these instances, the $25 application fee is not refunded.

    • Step 2: Psychiatric Evaluation

Dr. Kolp personally evaluates each application packet, and all applicants who qualify will continue on to the second phase of the assessment, the psychiatric evaluation. In the second phase, Dr. Kolp personally conducts a full, one-hour psychiatric evaluation for each applicant via Skype (or most other video chat programs of your choice). To participate in this process, each applicant must have access to a computer, high speed internet, and a web camera. Please feel free to Contact one of our patient care coordinators with any questions about technological requirements. Your initial $25 application fee is applied to the cost of your psychiatric evaluation resulting in an adjusted fee of $200. This fee must be paid in full prior to the psychiatric evaluation. Like your initial application fee, the price of your psychiatric evaluation will be applied to your total tuition upon acceptance. At the end of the evaluation, Dr. Kolp will either issue you an acceptance or contraindicate you for our program. Should your psychiatric evaluation contraindicate you for our program, the $225 psychiatric evaluation fee will not be refunded, and your medical record will be retained in our office for two years. Individual Transformation Program

For patients participating in our individual, outpatient psychotherapy program the full cost is $2,795.00 for ten weeks of psychotherapy and your ketamine session. A $1,000.00 deposit is due prior to beginning your telepsychiatry sessions via Skype with one of our licensed psychotherapists. Upon receipt of your deposit, we will send you a comprehensive, six week detoxification plan and welcome packet with information and guidelines to prepare you for your Individual Transformation Program . Full payment of your remaining balance, $1,570.00, is due prior to your ketamine session.

Group WELL Program

For clients participating in the residential, group psychotherapy program, they will be responsible for paying a $1,000.00 deposit toward the tuition upon selection. Upon receipt of your deposit, we will send you a comprehensive, six week detoxification plan and welcome packet with information and guidelines to prepare you for your Group WELL Program workshop. The total cost of the residential WELL Program workshop is $4,925.00. Full balance of your tuition, $3,700.00, is due no later than thirty (30) days prior to the start of your workshop. Medical records for all accepted applicants will be retained in our office permanently.

Methods of Payment

The Kolp Institute does not accept health insurance. We accept the following payment methods:

  • Personal Checks (Subject to verification prior to scheduling. All returned checks subject to $25 processing fee.)
  • Credit Cards (i.e. Visa, Mastercard, Discover, and American Express) via our secure checkout
  • Money Order
  • Cashier’s Check in U.S. Dollars

Refund Policy
Clients who are refused admission due to intoxication or any other condition that makes their admission inappropriate will not receive a refund.

Refunds will be also made for the following reasons:

  • Legitimate medical emergency (as ordered by a licensed physician)
  • Death in the family (proof required)

Refunds will be calculated as the pro-rated amount of the full program fee, based on the number of unused treatment days, minus the non-refundable deposit.

Clients who are discharged for any other violations of program rules will also not be provided a refund.
